What to include in your notice
- Your physical or electronic signature (or that of an authorized agent).
- Identification of the copyrighted work claimed to be infringed.
- Identification of the material to remove or disable, with enough detail to locate it.
- Your contact info: address, phone, and email.
- A good-faith statement that use of the material is not authorized by the owner, agent, or law.
- A statement that the information is accurate and, under penalty of perjury, you are the owner or authorized to act.
